First post here but I'm looking for those who have specifically had sarcoids treated via laser.

My horse has been battling a sarcoid for 5 months. We started with XXterra (3 nasty, painful, messy and pussy rounds from which sarcoid grew). It was getting worrying large and I decided to seek specialist attention after local vet was pushing me to keep going with XXterra.

I went to a specialist clinic who suggested laser surgery- luckily able to be done standing, and an injection of cisplatin beads. A lot (I repeat- a freaking lot) of beads were inserted. 5 FU cream was also applied for a week after surgery. Cost me a pretty penny (don't ask- no insurance and no other coverage so I'm down thousands).

my issue now is what to do with this raw, mince meat wound. The vet has said to keep applying the 5 FU now that stitches have been taken out. how in goodness is it EVER going to heal if I keep applying this stuff? It's a nasty inflammatory chemo cream and the wound is probably 15cm by 15cm, it's raw and a complete mess. I think there is a tension between allowing sarcoid back (which is not a good thing) and never letting this thing heal (which itself is a sarcoid risk- an open wound is an invite for the sarcoid to return).

What have other vets suggested post surgery? Remembering my horse already has long acting cisplatin beads in and surrounding the wound. They last up to 30 days as they are slow release so they are still operational. I read something about acyclovir- this to me would appear to be a good balance between letting the wound actually heal and preventing sarcoid from returning and obviously can be purchased over the counter. Any thoughts?

Mine had a wound that size after laser surgery, his was about the size of a large orange when it was taken off. No beads so not sure what they are. Eventually healed by itself. I think I applied the cream for about 10days which was a challenge. It was sore and bleeding for a while. No stitches either just left open. I put a fly sheet on him (cheap one) to keep it clean. I have him on Sarvoid now as well. He was on box rest for 6 weeks. He had originally had Liverpool cream and it came back with a vengeance, hence the laser, general anaesthetic as it was so bad. Good luck.

Thanks! Interesting yours didn't respond to liverpool cream. I don't think any sarcoid treatment has a 100% success rate.

I take it your boy is still sarcoid free? Fingers crossed mine goes the same way if so. Now 3 weeks post surgery and I'm really hoping this thing heals minus the sarcoid (still quite crusty and sore so I'm just applying wound gel to stop the crust getting too firm and pus pockets forming underneath).

Not personal experience, but a pony on my yard: He had a total of 42 sarcoids lasered at once, in armpits, groin and between his buttocks. Nothing was stitched, despite the wounds being deep through the adipose layer and into the muscle.

The only after surgery care he received was green clay directly on the wound areas twice a day.

Everything was slow to heal, obviously as the wounds were in highly mobile areas, but eventually he went back in work.

I think we are about 2 or 3 years post surgery, and he has had some recurrence, one or two sarcoidosis which have been banded, but generally he is fine. He is now 20.

I had one lasered ( no beads ) and it took fully four month s to heal others have taken much less time .
Fatty has had a lot of sarcoids off over the years .
I have two others where we removed them and they have never had another .
Fatty is somewhat immune suppressed because of other issues that’s a big factor you need to keep them as calm and stress free as possible .
You will need to keep the wound covered as the flys start that very important for your other horses .

Touch wood still ok. He was off work for a few months so does take time to heal. A huge wound. I could have put my whole hand in it. He could not walk for a week and refused to eat. The worst one was the size of a Jaffa orange and smelt like rotting meat. It was foul. It did still bleed in the summer but dried up 2 weeks after using sarvoid. May be a coincidence but I am keeping him on it.

My old horse had his taken off with liquid nitrogen when he was 6 and only one reappeared when he was 29. They do not use that treatment now.
