sheath swelling??

Can someone please tell me if their gelding has got the following?
Swelling in sheath- comes out march time and is more pronounced when weather is hot( he sweats round there from just standing there)
I thought it was dirty bits- as happened last year too, but I've cleaned and it's still just as swollen.

Equal on both sides in size, but not all the time.... Any info will be great :)

I might be wrong, but I think a swollen sheath can be a sign of insulin resistance, which might explain the seasonal occurence. Does your horse have any other symptoms, e.g. a cresty neck, history of laminitis? How is he weight wise, what is he fed and how much exercise does he get?
 
He is a 17.2hh warmblood just 5 years old, no laminitis so far (touch wood)
He gets ridden between 3-5 a week, muscular neck but not cresty

He is fed once a day and on recommended amount of balancer - 600g of equilibra 500 and one scoop of mollichaff condition, in winter he has this split between two feeds and one scoop of sugar beet in each feed too.
