Sheath swollen

Swollen sheath , not painful, has melmaomas but has been recently checked , salt water washes for thee days and re examine anybody else had this , horse out all day and worked also

My gelding has had this for the last two winters. His went down for the summer when he was on a track system 24/7 but once restricted to a small paddock and barn it has come up again. It does not seem to affect him in any way. I think the standing around for hours is one reason for it as he is no longer contiuously on the move ,ike in summer.

Does he have any cuts on his sheath?
Last summer my boy's sheath swelled up to the size of a rugby ball, at the back I found a cut which had become infected , vet visit resulted in him needing antibiotics, painkillers and saltwashes.
