Sore on top of tail

Puggy first developed this bald patch above his tail about five or six years ago now. It’s never been sore before - I put suncream on it in the summer but today it was itching him and I put some E45 cream on it, can back a couple of hours later and it’s now really sore. 🥺 I’m not sure if it’s a reaction the E45 cream or whether he had rubbed it. He doesn’t like it being touched. It’s not sunburn as it’s been raining here most of the day and this happened in the stable.

I did ask the Vet about it ages ago when it first appeared and they didn’t say much and didn’t seem overly concerned. I’m wondering if it’s just sensitive skin now he’s getting older (he’s 26). I have some steroid cream from when he had some bad fly bites this summer so might pop some of that on… just wondering if anybody else had seen anything like this before!

Dolly used to get this occasionally but hers was caused by sweet itch. The best thing I found was Equine America stop itch gel. I still use it occasionally now on the donkeys if they get a hot spot from a midge or horsefly bite. I have even used it myself once or twice for nettle rash, it works instantly, brilliant stuff.
