The way I understand it is that it is when the synovial membrane that lines a joint capsule starts producing fibrous villi that then interfere with the other components within the capsule.
There appears to be quite a bit written on synovial proliferation in humans but not so much on equines. If horses are the same as humans then there are apparently numerous causes (trauma can be one, yes) but that the type of proliferation can't be identified until a sample of the joint fluid is analysed.
