Just read this article and was reminded of a way X Rays are being marked and read by some Vets in US in assessing P3 and hoof capsule positions to clarify the picture and make X Rays more useful in guiding treatment and prognosis.
This is my understanding.
CE (Coronary band and P3 Extensor process) is/are marked, and the difference in height between them measured and used to guage the amount of distal decent (sinking of P3). The amount of decent has an effect on circulation and if severe and or long term, permanent damage can be done leading to difficulty growing a thick sole and even demineralization of P3.

Quote from above article

"How Consistent Are the Results?

There seems to be a “point of no return” or at least “much greater difficulty” when the CE measurements are at or beyond ¾-inch. For many years I had noticed this, and assumed that it was because more ripping and tearing had been done to the coronary papillae when the CE surpassed ¾-inch (instead of simply bending and distorting). This is not to say that larger CEs cannot be reversed, but that it is less likely, and will be a harder, longer road—for one thing, abscessing is very common in these cases, and that alone can make a horse want to give up.
"

Here are the other articles in the series to get a better idea.

Just thought it food for thought and may be part of the reason some horses have difficulty growing good, thick sole when everything is being done to help.

Hope I haven't confused everyone. ;)
