Vet charge for form filling?

I know they will charge something as it’s time.
How much would you expect or have you paid for a very simple (as in few minutes to fill in)
Insurance form?
 
Ours charge for direct insurance claims only (where insurer pays vet) - £21 for first claim, £10.50 for continuations. No charge for indirect claims, where you've paid the bill up front.
 
We charge £38 for a new direct claim (because not only are we doing the admin but we are also holding the debt, which can be hefty, and chasing it up if they are slow paying out). Capped at 3 charges per year. We don't currently charge anything for indirect claims ie client has paid us, has filled in their portion of insurance form, we just have to fill in vet portion and send history when requested. There is a strong possibility this will change soon, and there will be a charge, because it can still be quite time consuming.
