Vets can't figure out lameness - what to do next?

No by the hair line we mean the line where the hair ends, Ideally it should be wide with smooth contours, your horses is triangular which indicates the contraction of the palmar hoof.

That wasn't trimming really, that was taking under run long-term short shod hooves out of shoes
he was 19, lame 2/10 ish in front after bar shoes (which made them look better on the outside) and steroid into the coffin joint didn't help. Nothing much of note on xray other than negative palmar angle on both fronts (bullnosed externally to match)
Shoes came off in the March, 5 min walking on tarmac every day for a week, increased by 5 in every week.
Come week 6 he was a bit footy- particularly flat soles- and went into boots for road work for the next 6 months (we also have a long grassy bit so they used to come off for that), had first ridden canter in septmber was doing dressage in the november.

Moved to more challenging surfaces a year later (wiltshire bridleways and after a bit of transition time was fine on those too.

Hunted until he was 24, 28 now, retired but still sound on that hoof. Sadly my vet has now retired so I can no longer point out his nice hooves every time he visits ;).

He was trimmed, but once I took over that job only the toes needed it (he has a wonky action and therefore wear pattern) the back of the hoof sorted itself out.
FB tells me that photo was 5 years ago so still in full work at that point and 4 years since the shoes came off.
