if he is a horse that is keen on his feed he may still eat well even tho there is a possible tooth/abcess problem grumbling away, if you give antibiotics it could just damp the problem down whilst he is on them only for it to re occur when he comes off, so I would personally either get the horse dentist/your vet back to have a good inspection of the teeth.
 
I would agree with an abscess. Mine had exactly the same thing. Tended to come up every year around April! - Don't know why April, so I am waiting for it again soon. First 2 years I had vet out, x-rayed in year one and confirmed abscess but said if it got no worse then better to leave alone, They gave me a course of potassium iodide which I had to put in her food every day. Eventually after 2 to 3 months it disappeared. Year 2 vet out and decided to treat as previous year and see what happened, took 2 to 3 months to go again. Last year someone recommended aloe vera liquid which I put in her food every day, didn't have vet out and it disappeared within a month. This year I am tempted to give her aloe vera juice at the end of March and see whether it comes back or not. My mare was never bothered by it at all and I was able to adjust her bridle so that it didn't put any pressure on it.
 
I would hazard a guess as some kind of abcess, could be caused from a split tooth or some sort of infection in the mouth - a horse on our yard had something that looked exactly the same and the vet cut that open and a lot of puss came out, the owner has tried for 6 months to stop it but has now been told the tooth has to come out which will cost her 4K so make sure hes insured (shes not)! :)

4k to have a tooth removed... are they replacing it with a diamond lol

I have been rubbing aloe vera on the outside, but may try aloe vera in his feed.
Would it be best to have an x-ray or scoped?
It's not attached to the face if you know what I mean, you can move it about, further up the nose it's a hard lump.
