When does playfighting become serious?

How do you know when playfighting is actually becoming proper fighting?

My boy is kept out with 3 other geldings. They are all fairly young and often playfighting and he sometimes comes in with the odd little cuts and scraps.
But today he came in with a rather large flesh wound! I cleaned it up and think it looked a lot worse then it actually was.
The thing is, he is normally good as gold to turn out but when I tried to turn him out again he started throwing a tantrum as we walked up to the field and then refused to go through the gate! Once I got him in, he got chased off by one of the other horses.

I don't want to be one of these people to wrap my horse in cotton wool but I also don't want him to get a worse injury, or be unhappy!
They always chase him away from food/attention as he is the bottom of their pecking order but he normally comes back a minute later and all is fine. Today he stayed at the other side of the field to the rest of them!

Please send my boys some cuddles and carrots as he is a bit of a poof and can't stand up for himself ;)
