What a lovely type of horse with a very honest expression. To me she looks stiff in both hind legs and is being ridden in too quick a tempo for her to be able to keep her balance.
I would suggest riding her in a slower tempo to see if she becomes more level and also spend time using leg yield in walk and trot to help her become more supple. The unlevelness you can see in the video could just be due to lack of balance/suppleness but it would be best to have a full veterinary work up done to eliminate any serious pain factors.
