Hind bilateral PSD, prognosis?

Thank you everyone, I have been reading and following, just finding this quite hard to deal with.

Vet has offered lots of treatment options but recommended doing nothing and just pootle about hacking :(, I have another appointment lined up with him as I want to at least try, doesn't seem right not to give her every chance x

Alf had pretty bad PSD diagnosed at 15, with arthritis in both hocks to boot. He wasn't a candidate for surgery, so had various stuff done - I think that the thing that helped him most was PRP. I rehabbed him incredibly slowly, and have always been super careful about what surfaces I work him on (must be very stable and firm - if his feet sink into a surface more than 2cm, I won't ride him on it. He had Jim Blurton Hind Sports bar shoes on for a couple of years, and was always very sound in them.
Had he been a bit younger, I am pretty sure he would've come back into full work - he had several years of happily doing all the advanced movements - mostly when I asked for them - and feeling really strong and sound. He then had a fall on the road which resulted in arthritis in one of his knees, and he's just been a happy hacker since.

We didn’t scan mine to know what level of damage was there (vet said it could be the tiniest amount of damage but she could feel it lots or it could be a massive hole and it not bother her at all, so we just went based on nerve blocks) but there was a huge difference in her response to having the hind suspensories blocked, massive difference after the first set of steroids and a small improvement after the second set and I called it a day when they ran out.
But I also threw everything at her with the first set (water spa rehab, ulcer treatment and cartrophen too) so it’s a bit difficult to tell just what made the difference, but the 8-10 weeks after all that was the best she’d ever been!
She was another with multiple issues sadly.
