Only fallen off once on my boy (in the year I've had him). He tripped in front while cantering pretty fast in a field and drove me head first into the ground! I had nasty whiplash for days, couldn't turn my head at all.
But then I don't jump or anything anymore, and I have a horse who despite being green and occasionally spooky is very rider-friendly. So far (touch wood) he hasn't bucked, reared, napped, spun around, or ever tried to deliberately remove his rider, bless him.
