We have been told different versions, skin condition, scouring etc just curious to hear people's thoughts. Skin is raised and uneven, the scarring is fairly deep. No new fur growth in 18 months I've known him.

Skin infection that’s not been treated for a long time... then either cleared up on its own or got treated at a later date... that or a burn could do it

Possibly from falling onto a hard graveled surface when young, it looks as if there are parts that are deeper and the rest just the top layer friction burned which has stopped the hair growing, it can easily happen to a foal if it goes up and not released in time when being led, they often tip onto their backsides and a very young skin will probably be damaged more easily.

Are there any marks on its tail or other cheek? How old is he? Possible chemical burn.

Nothing on tail, bum other side has a scar from a serious bite... You can see from the shape and size it was another horse. Hes nearly 5

How interesting.
One of mine has lots of deep scar tissue in her bum, it looks like cellulite but has improved as she's developed more muscle.hers is very similar but without the hair loss, and on both sides.
We have concluded she's had some kind of crash in/with a trailer. (She's also very very afraid of trailers ;) )

Trailer thing would be a give away 🤣 he's very aggressive towards other horses, quite riggy and if he doesn't try to kill the other horse he annoys the living hell out of them so i questioned a bite that's got quite badly infected but it's just too clean around the edges and large i think. He's a rescue case and had lots of other physical issues and marks on arrival but this has never improved like the others have.

Interesting. One of mine ripped an A4 paper sized area of skin completely off his butt cheek, and it has healed back better than that, as the good skin eventually pulled together over the exposed flesh.

The dermis must have been damaged severely, without being removed. I'd go with a burn of some sort.

Its interesting as the tail doesn't appear affected at all, so if was a chemical or fire burn would be quite a specific area.

Road rash/burn injury maybe? If was in a carriage and fell could easily slide along that part without tail affected.

He wasn't backed but i know he was out showing as a foal at foot then nothing after as far as we know.

Maybe moving him and mare down a road and he slipped/fell?

The only one I've seen like it was a very deep abcess but it didn't affect as big an area as your boy has. It could also be scarring from repetitive scratching (like a sweet itch scar) but unlikely if tail not affected?

Working at a horse sanctuary for many years in the past Ive seen a lot but nothing the same as that. I've seen similar with pin worm left untreated but it's usually affected the tail too. But if it so happened that part of the horse got infected from constant rubbing and was left, it could cause a scar like that. ive seen a scar the same in looks and nearly square in shape that was from another horses bite. Just happened that this horse scars up easily Abd strangely. Like some people. My body's like a story book 😂

I reckon he's had pinworm that nobody's bothered to treat. Looks like he's rubbed himself raw on everything in sight, including wire, and been left with permanent scarring from months of damage on top of damage.

In that case why would the tail and other cheek be undamaged?

Sometimes a part will scar while another will not. I had a terrible motorbike accident years back and my whole shin and ankle was split open and skinned. Part has healed very well and you can't really seen damage but the other part (it got infected as was deeper) has left me very very badly scard. Unless you look closely at the skin on the tail head and other side with a microscope you may not see the damage. Not saying it is that just a possibility. If only they could talk!
