Lost Passport!

Unfortunatly in moving and clearing out i have stupidly lost my mares passport, it was a very basic passport as we have no breeding papers for her. She needs her jabs re doing as i am sure that we have gone over the time. Does any one know how you get a new passport re issued as i have never had to do so before?

Can you remember who issued it in the first place? I would give them a call and see what the process is for getting a replacement.
