What next?

I'm so sorry, but you've done your very best by him, and if even the vet agrees it's becoming the only sensible option then you can be sure you're doing the right thing. You've done so much to try and help him but he's clearly unhappy in day to day life, and leaving him to suffer wouldn't be kind. And remember to look after yourself - he won't know anything but you have every right to feel upset.
