The same thing could happen to any horse tied normally, not being tacked up, if it pulled back and the "string" or whatever didn't break for any reason.
It's not an accident confined to tacking up. Most of us tie up our horses for grooming, farrier, etc etc. It's a fact of horse owning that they need restraining.
This is still a tragic accident, regardless of any amount of what ifs and whys. I'm sure JJ has played over the events leading up to the tragedy over and over and doesn't need someone else heaping guilt on top of the overwhelming burden any horse owner in her position would already (and un-necessarily) have heaped upon themselves. Everything is so much clearer with the benefit of hindsight.
